Duties:


  • Provides clinical nursing, administrative and organizational skills in managing the continuity of care for the beneficiary populations.
  • Plans with the patient, the physician/provider, other healthcare providers, the payer and the community, to maximize healthcare response and quality, cost effective outcomes.
  • Participates in the identification of processes, systems, and practice metrics; and in determining measures of care outcomes for the population served.
  • Screens all potential case management clients within established timeframe after identification for appropriateness/benefit of case management services to client and/or institution.
  • Interview and counsel potential case management clients within established timeframe as to services needed to optimize current health/psychosocial status and benefits of case management services.

  • You will be required to provide proof of U.S. Citizenship.
  • Male applicants born after December 31, 1959, must complete a Pre-Employment Certification Statement for Selective Service Registration.
  • One year trial/probationary period may be required.
  • Applicants must comply with the Exceptional Family Member Program requirements as outlined in the DoD Instruction
  • This position requires that all health care providers must be able to obtain and maintain current Basic Life Support (BLS) training and certification prior to employment IAW AR 406
  • Current Advanced Cardiac Life Support or other advanced certification does not supersede BLS completion.
  • This position requires the incumbent to successfully complete and maintain credentialing and privileging requirements in accordance with Army Regulation 406
  • This position requires shift work to provide coverage on evenings, weekends, holidays, or special situations. Shifts may be rotated on a regularly scheduled basis.
  • This position requires Temporary Duty (TDY) or business travel approximately 25% of the time away from your permanent duty station.
  • Heavy lifting 20lbs.
  • This position requires the completion of a preemployment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position.

  • Basic Requirement for Nurse Clinical/Case Management
  • Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

  • Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination.
  • Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
  • Exemption: You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before. Applicants claiming this exemption must meet the requirement below.

  • Degree or diploma from a professional nursing program approved by the legally designated State accrediting agency at the time the program was completed by the applicant.
  • Active, current registration as a professional nurse in a State, District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
  • In addition to meeting the basic requirement above, to qualify for this position you must also meet the qualification requirements listed below:

  • Professional Nursing Experience is defined as: one year of professional nursing experience equivalent to the GS-11 grade level in the Federal service which include collaborating with a team of health care professionals to develop, implement, and monitor case management strategies to support efficient and cost effective outcomes for patients serviced; educating patients and members of a health care delivery team about treatment options and community resources; and conducting comprehensive assessments of patients' health needs in order to develop a plan of care.
